Pathways to funding, forms, computer systems, referral criteria, language and the education of health care professionals and birth workers have all been designed with cisgender\/heterosexual people in mind. When LGBTQ+ families navigate these systems, we often fall through the cracks.<\/span><\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">Many health care professionals and birth workers were educated during the Section 28 legislation, which made illegal the \u2018promotion of homosexuality as a pretended family relationship\u2019 during 1988-2003. Lack of education is one of the underlying issues behind the challenges LGBTQ+ people face. The lack of individualised language and terminology can further inequalities.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">Trans and non-binary people experience huge disparities in healthcare access and outcomes. In the UK society, hate crimes against trans and non-binary people have increased dramatically. Health care professionals need to ensure they are doing all they can to ensure trans and non-binary pregnant people have suitable access to individualised and humanising care.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">The Queer Birth Club offers LGBTQ+ <a href=\"https:\/\/linktr.ee\/thequeerbirthclub?fbclid=PAAabkbxduz0dsrPJwid6q6trPa36IO4Ke7zzaWleqD6_np0XbTKUGKufK_HE\">competency workshops<\/a> for birth workers and healthcare professionals, explaining language and terminology and statistics as well as LGBTQ+ history and case studies, allyship and more. Ask your NHS trust or educational setting to provide LGBTQ+ materials.https:\/\/uk.jkp.com\/products\/supporting-queer-birth<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">Excellent free insights into LGBTQ+ experiences include <\/span><i><span style=\"font-weight: 400;\">Seahorse, The Dad who Gave Birth <\/span><\/i><span style=\"font-weight: 400;\">(<a href=\"https:\/\/www.bbc.co.uk\/programmes\/m0008bxb\">BBC iPlayer<\/a>), <\/span><i><span style=\"font-weight: 400;\">Our Baby: A Modern Miracle <\/span><\/i><span style=\"font-weight: 400;\">(<a href=\"https:\/\/www.channel4.com\/programmes\/our-baby-a-modern-miracle\">Channel 4<\/a>) and the podcasts <\/span><a href=\"https:\/\/www.bbc.co.uk\/sounds\/brand\/p08cklsr\"><i><span style=\"font-weight: 400;\">Pride &amp; Joy <\/span><\/i><\/a><span style=\"font-weight: 400;\">and <\/span><a href=\"https:\/\/pod.co\/pride-in-birth\"><i><span style=\"font-weight: 400;\">Pride In Birth<\/span><\/i><\/a><span style=\"font-weight: 400;\">. There is also a growing base of research on LGBTQ+ conception, birth and the perinatal period, including journal articles at aims.org.uk.<\/span>&#8216;<\/p>\n<\/div>\n<\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-group rp-box rp-full-width has-global-padding is-layout-constrained wp-block-group-is-layout-constrained\">\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\">\n<h2 class=\"wp-block-heading\">Molly O&#8217;Brien on outdated medical models<\/h2>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:66.66%\">\n<p>&#8216;<span style=\"font-weight: 400;\">Medical language subjugates women. In textbooks and hospitals around the UK, when a woman faces difficulties in labour she might hear \u2018failure to progress\u2019. Or the more derogatory \u2018inefficient contractions\u2019, \u2018inadequate pelvis\u2019, even \u2018insufficient maternal effort\u2019.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">Instead of the symptoms, we should examine the root causes of difficulties during birth. I teach that these are linked to biomechanics. We must listen to what women say about their bodies. Health professionals treat them as if their body is not very well made or tell women they\u2019re risking their babies\u2019 lives if they don\u2019t listen to medical advice.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">In the UK, we are still working on the 1938 <a href=\"https:\/\/nursing.uic.edu\/news-stories\/faculty-call-for-an-end-to-pelvis-classification-system-in-new-paper\/#:~:text=The%20Caldwell%2DMoloy%20system%20was,with%20a%20different%20birth%20outcome.\">Caldwell and Moloy<\/a> model on pelvic types. It defines the \u2018normal\u2019 type and mechanisms of how the baby passes through the pelvis. The model is racist. It\u2019s based on a European study. What about women from other parts of the world?<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\"><a href=\"https:\/\/royalsocietypublishing.org\/doi\/full\/10.1098\/rstb.2020.0024\">Studies show<\/a> that pelvic shapes exist on a spectrum. Relying on an outdated model influences clinical behaviour, placing judgement on women, especially black women, and encourages unnecessary medical interventions \u2013 induction, C-sections, use of forceps \u2013 which impact the woman\u2019s and baby\u2019s body post-birth.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">People end up carrying trauma with them, maybe for the rest of their lives. Some physical harm is life changing. <a href=\"https:\/\/www.theguardian.com\/society\/2022\/apr\/07\/it-seems-if-you-have-a-baby-you-should-expect-to-be-in-pain-the-women-living-with-postpartum-incontinence\">Pelvic prolapse<\/a>, for instance, is common. Instead of support, we have advertisers selling lacy pants so we can \u2018feel sexy\u2019 while suffering incontinence as a result of a birth. Women deserve more.<\/span>&#8216;<\/p>\n<\/div>\n<\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-group rp-box rp-full-width has-pale-1-background-color has-background has-global-padding is-layout-constrained wp-block-group-is-layout-constrained\">\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\">\n<h2 class=\"wp-block-heading\">Devon Price on hormone damage<\/h2>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:66.66%\">\n<p>&#8216;<span style=\"font-weight: 400;\">A battle is playing out in UK <a href=\"https:\/\/www.theguardian.com\/society\/2021\/sep\/17\/appeal-court-overturns-uk-puberty-blockers-ruling-for-under-16s-tavistock-keira-bell\">courts<\/a>, <a href=\"https:\/\/www.telegraph.co.uk\/news\/2022\/07\/28\/nhs-faces-crackdown-giving-puberty-blockers-children-tavistock\/\">parliament<\/a> and the <a href=\"https:\/\/www.theweek.co.uk\/news\/science-health\/957525\/tavistock-clinic-closure-trans-rights-debate\">NHS<\/a> over whether transgender teens can <a href=\"https:\/\/www.theguardian.com\/world\/2021\/jan\/19\/high-court-grants-leave-to-appeal-to-uk-gender-identity-service\">meaningfully consent<\/a> to taking puberty blockers. The argument behind this curtailing of trans people\u2019s rights is that the medications\u2019 effects are not sufficiently known and may have permanent effects that later become unwanted.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">I am a transgender person. I felt pressured into taking hormones with poorly studied and at-times irreversible effects. It was the birth control I was put on at 18, not the hormone replacement therapy (HRT) I take now.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">My friends were pushed onto the pill by anxious parents, by boyfriends, by doctors saying it would manage painful and irregular periods, or just to get rid of body hair or <a href=\"https:\/\/twitter.com\/tiktikitomtom\/status\/1378739864523378695\">spots<\/a>. It had significant effects on my body, some permanent, hyper-feminising it in ways I hated: my breasts ballooned, my body became softer, my face rounder, my body hair lighter.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">I was despairing a week before my period, every single month. People have reported negative mood effects from hormonal birth control since the 1970s. Research attesting to that only found traction in the 2010s. To this day many medical professionals <a href=\"https:\/\/news.northwestern.edu\/stories\/2020\/11\/birth-control-does-not-cause-depression\/\">deny links<\/a> between c<a href=\"https:\/\/ajp.psychiatryonline.org\/doi\/abs\/10.1176\/appi.ajp.2017.17060616\" target=\"_blank\" rel=\"noreferrer noopener\">ontraceptives, depression and suicide<\/a>, despite the <a href=\"https:\/\/jamanetwork.com\/journals\/jamapsychiatry\/fullarticle\/2552796\">evidence<\/a>.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">The pill has also changed countless lives for the better. I would never advocate for restricting access to it. The problem isn\u2019t its existence or accessibility. I took it because I lived in a sexist, coercive society that hadn\u2019t listened to the people clamouring for better options, for decades. We still need better research and to listen to users. The informed consent model that works well for trans people accessing hormones, like me, could offer a blueprint for respectful engagement with teens looking into contraceptives.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400;\">We also need more research into providing quality healthcare to trans people. Doctors generally don\u2019t care about our experiences or our long- term health outcomes. They get away with mistreating trans patients, particularly trans women, because<\/span> <span style=\"font-weight: 400;\">we are so roundly dehumanised. Restricting access to HRT and hormone blockers \u2013 denying our agency \u2013 won\u2019t fix that. Women living in the most deprived areas are twice as likely to die in pregnancy and childbirth as those in the most affluent parts of the UK. <a href=\"https:\/\/www.birthrights.org.uk\/campaigns-research\/racial-injustice\/\">Black women are four times more likely to die<\/a>, and Asian and mixed-race women twice as likely as white women<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400;\">A <a href=\"https:\/\/www.rcm.org.uk\/news-views\/rcm-opinion\/2022\/maternity-services-are-highlighted-as-an-area-of-special-concern-in-cqc-report\/\">2022 Care Quality Commission\u2019s report<\/a> highlights England\u2019s maternity services as an \u2018area of special concern\u2019. Two in five maternity service units are \u2018not good enough\u2019<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400;\">The UK ranks <a href=\"https:\/\/factcheckni.org\/topics\/economy\/is-full-time-paid-maternity-leave-in-the-uk-one-of-the-lowest-in-europe\/\">ninth lowest of 33 European countries<\/a> for \u2018full-rate equivalent\u2018 paid maternity leave<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400;\">Due to poor IVF funding, <a href=\"https:\/\/www.oecd.org\/els\/soc\/PF2_1_Parental_leave_systems.pdf\">NHS England and Wales trusts often cannot offer <\/a>the recommended cycles per person. Some refuse to treat women over 35, those who cannot prove they are in a \u2018stable relationship\u2019, or couples where one partner has a child from a previous relationship.
